Please fill out both forms, as completely as possible, and bring the completed forms with you to your office visit . You may also fax them to us, drop them off at our office or mail them to us. If you wish to fax your forms, please fax a copy to each of our offices (Princeton fax number is 609-921-8096. Cranbury fax number is 609-655-3003). Sending a copy to both of our offices assures that we have your information readily available in each office. This will allow us to enter your information prior to your visit and shorten the length of time you spend in our office.

Additional items to bring to every visit:

1. Your current health insurance card(s) . Your insurance company requires that we see your cards at each visit. Even if your insurance company may not have changed since your last visit, we need to confirm that our information is up to date.

2. Your insurance referral . If your insurance requires a referral, your insurance company requires that we MUST have your referral prior to your seeing the doctor.

3. Your photo ID . A Driver's license is the preferred photo ID.

4. Your current medication list with the dosage and frequency of each medication. Please do not rely on old medications lists you have given us in the past. Medications and dosages may have changed since your last visit and we want to confirm that we have the most current information.

5. Your drug allergy list . You can become allergic to something at any time, so we check that at every visit.

6. Your co-pay. It is a requirement of your insurance company that you pay your co-pay prior to seeing the doctor.

7. Any reports and/or films that may be relevant to the reason for your visit. This would include - hearing tests, CAT scan, MRI scan, X-ray study, surgical operative reports, consultation reports, lab results, blood tests, pathology reports or culture results.
